How to Complete the Membership Form below.
1st member: Is the name of the member that the pass will be issued to. It is usually a child 2 or older.
Additional members: Additional members can include members in your family, or any other person that you want added to your pass. Please enter the names in the spaces provided below. Please include first and last names when they are different from the 1st named member.
Unnamed member : The unnamed member can be a different person each visit. Limit one unnamed member per card.

Once we receive your online order form, we will contact you to complete the processing
of your membership and mail your pass. Please complete a separate form for each pass
that you are purchasing.
